Consumer Credit Reporting Consents and Notices

In compliance with the State in which the subject property is located, and Federal Laws, you are hereby notified that a Consumer Credit Report and/or Investigative Consumer Report may be obtained for use in evaluating our decision to accept your personal a guarantee for the home improvement project on the subject property*, the extension of credit in the form of providing Company resources necessary to complete the project in most efficient way possible, guarantee of the Assignment of Insurance Claim Benefits Agreement, retention and/or for collection purposes in the event of default of any of the effective Agreements between the you and the Company.

Inquiries may be made in considering your application , and the report obtained by Company may contain public/ semi-public or private information, identification information, credit information, DMV records or other information, which could adversely affect your potential for an association with us.

This document is to serve as your written consent/ authorization for State Restoration Services, Inc. and/or any of its affiliates, agents, officers, employees, consultants, successors, and all other related persons, firms, corporations, associations, or partnerships Including but not limited to Consumer Reporting Agencies, related or unrelated firms, public, private, government, law enforcement and/or other agencies and/or persons to release information in response to these inquires, and release all of the above from any and all liability in responding to such inquiries. I also fully indemnify any and all of these associated with this consent against any and all direct, indirect, and/or consequential claims, actions, causes of action, demands, rights, damages, costs, loss of service, loss of rents, expenses, and compensation whatsoever that I may now have or may have in the future as a result of any foreseen or unforeseen consequences which might arise in the course of process of the use of this consent.

You have the right to make a direct written request to obtain copies of any reports, which may have been provided by one, or more of the following Consumer Reporting Agencies, which may have contributed to the compilation of the Consumer Credit Report, and/or Investigative Consumer Report (copies of privacy policy available on each website).
